Solving access and equity issues at the community level in cancer care

The time for talking about health equity is over. We need to take collective action.

Connecting Life to Care is a program uniquely focused on solving the challenges faced by community oncology providers and the diverse patient populations they serve.

The opportunity

Work collectively to reduce health disparities by giving a voice to diverse patient populations

Why it matters

80% of cancer patients receive care in community settings – to effect change we must meet patients where they are and take a data-driven approach

The goal

Evidence-based programs and policies that have the greatest impact for cancer patients in the near and longer term

Accelerating progress through practical solutions and data-driven innovation

Connecting Life to Care is bringing actionable solutions and resources to community oncology while collaborating with senior healthcare leaders, oncologists, patient advocates, and policy makers from across the country to:

  • Enhance health equity initiatives at a patient-level with innovative solutions, purpose-built for oncologists and the diverse communities they serve

  • Analyze patient-reported data on the Social Determinants of Health (SDoH), patient priorities, and preferences influencing patient access and outcomes

  • Identify opportunities to deploy sustainable, evidence-based programs that expand access to high quality care to all cancer patients

Ask the Expert video series

The recent introduction of new CMS reimbursement codes for Social Determinants of Health (SDoH), Community Health Integration (CHI), and Principal Illness Navigation (PIN) brings both opportunities and challenges for community oncology. This series aims to address the most frequently asked questions and provide clear, actionable answers to help oncology practices navigate these changes confidently
